Telephonic care management
Providing patient support and care coordination

Required Qualifications

Active RN license in Oregon
Care management experience

Certification Required

Licensed Registered Nurse (RN)

Schedule

Shift: 8 AM - 5 PM MST
Days: Monday - Friday

Dress Code: Telecommuter (No scrubs required)
